SSMT and Bien Chez Soi partner to fix gaps in home care

In response to the ongoing strain on Quebec’s public health network and mounting delays in access to home care services, a new partnership has been formed between Clinique SSMT and Bien Chez Soi Outaouais. The initiative seeks to address growing gaps in care for vulnerable residents, particularly the elderly and those with loss of autonomy, by streamlining and expanding access to both clinical and at-home support services.


The collaboration is designed to ensure faster interventions, better coordination between providers, and improved continuity of care directly in patients’ living environments. The partnership is intended to accelerate access to services and reduce the burden on families who often must coordinate with multiple providers to manage care.


The model enables direct communication between the two organizations. When Clinique SSMT identifies a need for home support, the information is shared with Bien Chez Soi to initiate an intervention. Conversely, when Bien Chez Soi staff recognize a need for medical or nursing care, patients are referred to SSMT, either for in-clinic or at-home services. This system is meant to reduce delays and avoid fragmented care pathways that can place added strain on patients and caregivers.


The announcement follows the release of Mieux chez soi, a new provincial policy focused on enhancing home care and supporting aging in place. The two organizations say their approach is aligned with the policy’s objectives, particularly by supporting family caregivers and improving care delivery within the home.

 

“By combining our expertise in home support and in-home nursing care, we aim to simplify the patient journey and support families where the needs are greatest,” said the Clinique SSMT team in their statement.
